GROUP PARTICIPATION & WAIVER OF LIABILITY AGREEMENT

Thank you for participating in a Main Street Counseling (“MSC") Group/bootcamp. This is a support group and although this is a counseling practice this group is NOT therapy services. The information presented during the group is educational in nature and provided as general information and is not medical or psychological advice. MSC and its leaders, volunteers, members and representatives, are not responsible for any injury, loss or damage of any kind whatsoever sustained by any person or their property while participating in sessions, or events with MSC groups/bootcamps and all related activities associated with MSC groups/bootcamps, including injury, loss or damage.

ASSUMPTION OF RISKS

In consideration of MSC groups/bootcamps allowing my participation in events, conversations, and activities, with MSC groups/bootcamps and all related activities associated MSC’s groups/bootcamps, including participation in the group and all activities related to the group, I acknowledge that I am aware of the possible risks, dangers and hazards associated with participation in the activities including any injury, loss or property damage.

RELEASE OF LIABILITY and AGREEMENT

In consideration of MSC allowing my participation in the group, I agree:
1. TO ASSUME and ACCEPT ALL RISKS arising out of, associated with or related to my participation in the New Mom’s Group activities, events, and discussions.
2. TO WAIVE and RELEASE MSC from any and all liability for any loss, damage, injury or expense that I or my child may suffer, or that my next of kin may suffer as a result of my/my child’s participation in the activities, events, and discussions due to any cause whatsoever.
3. TO INDEMNIFY and HOLD HARMLESS MSC from any and all liability for any damage to the personal property of, or personal injury to, any third party resulting from my/my child’s participation in the activities.
4. TO INDEMNIFY and HOLD HARMLESS MSC from any and all claims, demands, actions and costs for any loss, injury, damage or expense whatsoever that might arise out of my or my child’s participation in the Activities.
5. If any portion of this agreement is her to be invalid, it is agreed that the balance of the agreement shall continue in full force and affect. This agreement shall be binding upon you, your heirs, family, legal representative, successors and assigns.
6. This agreement constitutes the entire and final agreement between the parties and supersedes any prior oral or written agreements or understandings.

 

Acknowledgment of Participant:

I hereby authorize and consent to my involvement, or my childs involvement, in MSC groups/bootcamps, including any discussions, events, or activities.

I UNDERSTAND THAT THIS IS A LEGAL AGREEMENT that is binding upon myself and my heirs, executors, administrators, successors and assigns. I HAVE READ AND UNDERSTAND THE TERMS OF THIS AGREEMENT and I ACKNOWLEDGE THAT by signing this agreement voluntarily, I am agreeing to abide by its terms.

 

Payment: 

Payment must be made in full at registration to hold your spot for this group/workshop. Payment options below:

2. Zelle "Mainstcounselingsolutions@gmail.com" - must be paid 24 hours prior to start date.

In the memo of zelle please make sure to list the participant's name and full group/workshop name.
